Dosage and Administration

Doses depend on level of pain and tolerance. Larger doses may be required during chronic therapy. The usual starting dose is 1-2 mg subcutaneously or intramuscularly every 4 to 6 hours as necessary for pain control. The dose should be adjusted according to the severity of pain, as well as the patient’s underlying disease, age, and size. Patients with terminal cancer may be tolerant to opioid analgesics and may, therefore, require higher doses for adequate pain relief. Intravenous or subcutaneous administration is usually not painful.

Should intravenous administration be necessary, the injection should be given slowly, over at least 2 to 3 minutes, depending on the dose. A gradual increase in dose may be required if analgesia is inadequate, tolerance occurs, or if pain severity increases. The first sign of tolerance is usually a reduced duration of effect. Patients with hepatic and renal impairment should be started on a lower starting dose

Contraindications/Precautions

Contraindicated in: Hypersensitivity; Some products contain bisulfites and should be avoided in patients with known hypersensitivity; Severe respiratory depression (in absence of resuscitative equipment) (extended-release only); Acute or severe bronchial asthma (extended-release only); Paralytic ileus (extended-release only); Acute, mild, intermittent, or postoperative pain (extended-release only); Prior GI surgery or narrowing of GI tract (extended-release only); Opioid non-tolerant patients (extended-release only);OB, Lactation: Avoid chronic use during pregnancy or lactation.

Use Cautiously in: Head trauma;qintracranial pressure; Severe renal, hepatic,or pulmonary disease; Hypothyroidism; Seizure disorder; Adrenal insufficiency; Alcoholism; Undiagnosed abdominal pain; Prostatic hypertrophy; Biliary tract disease (including pancreatitis); Geri: Geriatric and debilitated patients may be more susceptible side effects; doseprecommended.

Adverse Reactions/Side Effects

CNS:confusion, sedation, dizziness, dysphoria, euphoria, floating feeling, hallucinations, headache, unusual dreams. EENT: blurred vision, diplopia, miosis. Resp: respiratory depression. CV: hypotension, bradycardia. GI:constipation, dry mouth, nausea, vomiting. GU: urinary retention. Derm: flushing, sweating. Misc: physical dependence, psychological dependence, tolerance.

Patient/Family Teaching

  • Instruct patient on how and when to ask for pain medication. Do not stop taking

without discussing with health care professional; my cause withdrawal symptoms

if discontinued abruptly after prolonged use.

  • Advise patient that hydromorphone is a drug with known abuse potential. Protect

it from theft, and never give to anyone other than the individual for whom it was

prescribed.

  • May cause drowsiness or dizziness. Advise patient to call for assistance when ambulating or smoking. Caution patient to avoid driving or other activities requiring alertness until response to medication is known.
  • Advise patient to notify health care professional if pain control is not adequate or if side effects occur.
  • Advise patient to change positions slowly to minimize orthostatic hypotension.
  • Instruct patient to avoid concurrent use of alcohol or other CNS depressants.
  • Instruct patient to notify health care professional of all Rx or OTC medications, vitamins, or herbal products being taken and consult health care professional before taking any new medications.
